| The revised Trademark Law and the Regulation on the Implementation of the Trademark Law have come into force on May 1, 2014. To regulate the recordation of trademark agencies and facilitate trademark agencies' provision of agency services, in accordance with the provisions of the Regulation on the Implementation of the Trademark Law, the Trademark Office has developed the format of documents for the recordation of trademark agencies, which is hereby announced. | | 修订后的商标法及商标法实施条例已于2014年5月1日施行。为规范商标代理机构备案工作，方便商标代理机构开展代理业务，根据商标法实施条例规定，商标局制定了商标代理机构备案书式，现予公布。 |

| | 1.商标代理机构办理备案，适用本书式。商标代理机构应当按照要求如实填写，不得擅自修改格式。备案表应当打字或者印刷。 |
| | 2.商标代理机构应当填写名称并在右下方“商标代理机构（章戳）”栏和“负责人（签字）”栏盖章和签字。 |
| | 3.商标代理机构名称应当与“商标代理机构（章戳）”处所盖章戳及所附主体资格证明文件中的名称一致。 |
| | 4.商标代理机构地址应当按照主体资格证明文件中的住所或者地址填写，主体资格证明文件中的住所或者地址未冠有省、市、县等行政区划的，应当在填写商标代理机构地址时，在该住所或者地址前注明相应行政区划。 |
| | 5.负责人应当与所附主体资格证明文件中的“法定代表人”或“负责人”一致。 |
| | 6.身份证明文件编号应当与所附主体资格证明文件中的证件编号一致。 |
| | 7.须附送盖有登记主管部门章戳的主体资格证明文件复印件或者经公证的主体资格证明文件复印件，并加盖商标代理机构章戳，以及经办人有效身份证明文件的复印件。 |
| | 8.商标代理从业人员名单，应当包括商标代理机构负责人和商标代理从业人员的姓名、身份证号码、联系电话、签名。负责人信息应当填写于商标代理从业人员名单首位。附表填写不下的，可按本格式另附纸张填写。 |
| | 9. 办理商标代理机构备案事宜，请详细阅读“商标代理机构备案办理须知”（www.saic.gov.cn）。 |
