| To regulate the administration of population health information, promote the interconnection, interworking, and shared use of population health information, and carry forward the scientific development of the health and family planning cause, in accordance with the relevant laws and regulations, the National Health and Family Planning Commission (“NHFPC”) makes research and develops the Measures for the Administration of Population Health Information (for Trial Implementation), which are hereby issued to you for your compliance and implementation. | | 為規範人口健康信息的管理工作，促進人口健康信息的互聯互通和共享利用，推動衛生計生事業科學發展，我委按照相關法律法規，研究制定了《人口健康信息管理辦法（試行）》。現印發你們，請遵照執行。 |

| Article 1 These Measures are developed for purposes of regulating the administration of population health information, promoting the interconnection, interworking, and shared use of population health information, and carrying forward the scientific development of the health and family planning cause. | | 第一條 為規範人口健康信息的管理工作，促進人口健康信息的互聯互通和共享利用，推動衛生計生事業科學發展，制定本辦法。 |
| Article 2 These Measures shall apply to such work as the collection, administration, use, and safety and privacy protection of population health information involved in medical, health and family planning service agencies at various levels. | | 第二條 本辦法適用于各級各類醫療衛生計生服務機構所涉及的人口健康信息的采集、管理、利用、安全和隱私保護工作。 |
| Article 3 For the purpose of these Measures, “population health information” refers to such population health information as basic population information and medical and health service information originating in the service and administration process of medical, health and family planning service agencies at various levels in accordance with the laws and regulations of the state and their functions and duties. | | 第三條 本辦法所稱人口健康信息，是指依據國家法律法規和工作職責，各級各類醫療衛生計生服務機構在服務和管理過程中產生的人口基本信息、醫療衛生服務信息等人口健康信息。 |
| Electronic population health information that conforms to the Law of the People's Republic of China on Electronic Signature and other relevant laws and regulations shall have the same legal effect as the document in hardcopy. | | 符合《中華人民共和國電子簽名法》等有關法律法規規定的人口健康電子信息，與紙質文本具有同等法律效力。 |
| Article 4 The administration of population health information shall be subject to overall planning, uniform standards, territorial administration, consistency between power and responsibilities, safety guarantee, and convenience and efficiency. | | 第四條 人口健康信息管理工作應當統籌規劃、統一標准，屬地管理、責權一致，保障安全、便民高效。 |
| Article 5 Administrative departments of health and family planning (including administrative departments of traditional Chinese medicine, here and below) of the people's governments at or above the county level are competent departments of population health information. The NHFPC shall be responsible for formulating the national development plans and administration norms for population health information, and coordinating and guiding the national administration of population health information; and the administrative departments of health and family planning at or above the county level shall be responsible for carrying forward, guiding, and supervising the administration of population health information within their respective administrative regions. | | 第五條 縣級以上人民政府衛生計生行政部門（含中醫藥行政部門，下同）是人口健康信息主管部門。國家衛生計生委負責制訂全國人口健康信息發展規劃和管理規範，統籌指導全國人口健康信息管理工作；縣級以上地方人民政府衛生計生行政部門負責推進、指導、監督本行政區域人口健康信息管理工作。 |
| Medical, health and family planning service agencies (including institutions of traditional Chinese medicine, here and below) at various levels shall be responsible for the collection, use, administration, safety and privacy protection of population health information and they are entities in charge of the administration of population health information (hereinafter referred to as "entities in charge"). | | 各級各類醫療衛生計生服務機構（含中醫藥服務機構，下同）負責人口健康信息的采集、利用、管理、安全和隱私保護，是人口健康信息管理中的責任單位。 |
| Article 6 To collect, use and administer population health information, entities in charge shall, in accordance with laws and regulations, follow the principle of observing medical ethics, guarantee information safety, and protect personal privacy. | | 第六條 責任單位采集、利用、管理人口健康信息應當按照法律法規的規定，遵循醫學倫理原則，保證信息安全，保護個人隱私。 |
